When you download and print this application, please note the following:
Regarding References: You will need to provide business-sized envelopes to each person writing a reference. Please ask each person to seal his/her reference inside the envelope and sign the back flap before returning to you for inclusion in your application packet.
When mailing CTS your application: The application form, essays, references and any other supporting documentation should be mailed to Chicago Theological Seminary in one envelope. Please mark "Application Enclosed" on the outside of the envelope.
